Trip Visitation Form for WNS Study

The agencies involved in the White Nose Syndrome research are requesting that people who have visited:
 
Knox Cave, Schoharie Caverns, Gage Caverns, and Hailes Cave
 
since January 2007 submit information about all your caving trips since then so that they can better understand and track the potential spread of WNS as it relates to caving.
 
Please submit whatever data you have on your caving trips after January 2007 using the form below to help with the studies on WNS.
 
The main interest is in visited caves outside of the immediate NY/Schoharie area, so that it can be studied whether WNS is or has been transmitted by humans. Location and approximate dates are the most sought after information.
 
If you noticed anything during a trip that appeared to be WNS or concentrations of dead bats, please also provide any (or all) of the following: USGS, DATUM, GPS Coordinates. NAD83, Degree/Minutes/Seconds (DMS) is the preferred datum and coordinate system of the US Fish and Wildlife Service.
